This LCD revision is effective for claims processed on or after October 4, 2011, for services rendered on or after January 1, 2010.
What was revised?
The local coverage determination (LCD) for paravertebral facet joint blocks was most recently revised on June 14, 2011. Since that time, another revision took place. Under the “Limitations” section and in the “Coding Guidelines” attachment, the verbiage was revised to clarify that fluoroscopy is an inclusive component of CPT codes 64490-63395 and not paid separately.
In summary: Florida’s revised LCD L29252 to state: “Per CPT Imaging guidance (fluoroscopy CT) and any injection of contrast are inclusive components of 64490-63395” for services rendered on or after January 1, 2010.
